3 to 8 Years Follow-up of Total Knee Arthroplasty




Abstract
Long-term studies of total knee arthroplasty(TKA) have confirmed reliable relief of pain and maintenance of function. We evaluated retrospectively 104 TKAs which were performed between January 1985 and April 1993 and followed up more than three
years.
There were 79 knees of osteoarthritis(OA), 21 knees of rheumatoid arthritis(RA) and 4 knees of septic knee sequelae. Among them, 38 TKAs were performed with cemented prosthesis, 21 TKAs with cementless prosthesis and 45 TKAs with hybrid
prosthesis.
The follow-up period was 4 years and 7 months9range, 3 to 8 years). Using the knee rating scale of the Hospital for Special Surgery, American Knee Society Rociety Roentgenographic Evaluation and Scoring System and Tew¢¥s method of analyzing the
reusults of TKAs, we evaluated clinical and roentgengraphic results of TKAs followed up more than 3 years and method of fixation.
@ES The results were as follows:
@EN 1. Pain scores improved and there has no significant difference between OA group and RA group.
2. ROM of the knees improved in OA group more than RA group postoperatively. Increase of ROM in OA group was statistically significant from one to three years postoperatively and decrease of ROM in RA group was statistically significant from
three to five years postoperatively.
3. Knee scores were higher in OA group than RA group postoperatively but there was no significant difference among cemented, cementless and hybrid TKA group.
4. The radiolucent lines wider than 2mm were more frequently observed in A-P roengenograph of tibial component but there was little correlation with clinical results.
5. Cumulative success rate was 86 percent at 8 years provided success in judged by whether the prosthesis is still in situ without severe pain and annual failure rate was higher at 3 to 4 years postoperatively.
